Overview

When used as a space maintainer (rather than an anchorage device), the Nance holding arch preserves bilateral upper arch space after premature loss of primary molars. The device prevents the first permanent molars from drifting mesially, maintaining the space needed for erupting premolars. Its palatal acrylic button provides tissue-borne resistance so the appliance does not rely solely on the molar bands to resist mesial drift forces. It is the standard bilateral maxillary space maintainer in pediatric orthodontic and pedodontic practice.

Clinical & technical

Mechanism of action

Same as Nance button (Nance button) — the molar bands and palatal button work together to resist mesial molar migration. In the space maintenance context the forces are lighter (passive drifting vs. active mechanics), so the button contact is sufficient to hold position. The appliance is passive and delivers no active force — it simply blocks space.

Indications & case selection

Bilateral premature loss of maxillary primary molars (most commonly second primary molars before eruption of second premolars); preservation of Leeway space during late mixed dentition; holding arch space while awaiting eruption of successaneous teeth; post-extraction space maintenance when bilateral symmetry is needed. Preferred over two separate unilateral band-and-loop maintainers because one appliance covers both sides.

Contraindications & limitations

Not appropriate for unilateral loss — a band-and-loop (see Band and loop space maintainer) is preferred for single missing teeth. Palatal tissue irritation from the button requires monitoring. Cannot guide eruption of unerupted teeth — does not replace a distal shoe appliance when the permanent first molar is unerupted. Must be removed or replaced as permanent teeth erupt and the arch changes.

See Nance button for full fabrication details. When used as a pure space maintainer, the button can be slightly larger to maximize tissue contact and passive resistance. Ensure smooth button edges and adequate clearance from soft tissue at delivery.
