The Thumb-Tongue Habit Appliance is a fixed habit-interrupting appliance that addresses both digit sucking and tongue thrusting simultaneously in a single bonded appliance. Unlike banded habit appliances (which require molar bands), this appliance is bonded directly to the posterior teeth using composite resin, eliminating the need for band impressions and improving patient comfort. The design incorporates elements to deter digit insertion (a raking or spiked barrier at the palate) and block tongue thrust (a palatal crib). CFL prices this as the Bonded Habit Breaker — the "bonded" designation distinguishes it from the banded tongue crib (Tongue crib).
The bonded palatal barrier sits behind the upper anterior teeth and extends into the palate. When the thumb or finger is inserted, it contacts the barrier and cannot reach the palatal vault — the reward of the habit (negative intraoral pressure and palatal contact) is eliminated. Simultaneously, the crib wires block the tongue from thrusting forward. Both habit deterrents operate continuously 24/7 without compliance. Most patients extinguish the habit within 2–3 months.
Combined thumb/finger sucking AND tongue thrusting habit; patients where a banded appliance is not preferred (non-banding practices; very young patients); cooperative patients who accept the bonded design; anterior open bite from combined digit + tongue habit; patients who have failed removable habit appliance treatment.
Bonded design means the appliance cannot be adjusted as easily as a banded version. If a bond breaks, chairside rebonding is required. Hygiene around bonded attachments requires monitoring.

Bonded attachment pads must be precisely contoured to the lingual/occlusal surface of the bonding teeth. The wire framework is soldered to the pads. The overall design must balance effectiveness (wire coverage) with minimal bulk. CFL's Bonded Habit Breaker is the standard pricing for this design.
