Overview

Mechanism of action

Incline plane (Class III): Lower incisors contact an angled acrylic ramp on the upper retainer during closure, producing a distal force on the lower anterior teeth and a forward force on the upper anteriors — correcting anterior crossbite. Most effective in mixed dentition (growing patients with an accommodative bite). The incline plane angle (typically 45–60°) determines the force direction.

Anterior bite ramp (Class II/deep bite): Upper anterior coverage with a bite ramp that contacts the lower incisors and holds the mandible in a slightly forward position, preventing the lower anteriors from occluding deeply. Simultaneously opens the posterior bite to allow posterior eruption (bite plate function).

Dual bite blocks: Bilateral blocks that prevent full posterior closure, used for vertical dimension control or combined anteroposterior + vertical bite correction.

Indications & case selection

Anterior crossbite in mixed dentition where the incline plane can redirect lower incisor eruption; Class III tendency with dental (not skeletal) anterior crossbite; deep overbite maintenance during retention; mild Class II correction during the retention phase; cases where a passive retainer would lose bite correction already achieved in treatment.

Contraindications & limitations

Incline plane correction only works in growing patients with accommodative bite — adult anterior crossbites are skeletal and require orthognathic surgery or comprehensive retreatment. High-angle patients should avoid anterior bite ramps (worsens vertical). Compliance-dependent.

The incline plane angle must be specified by the ordering doctor — standard is 45–60° from horizontal. The acrylic ramp must be smooth and polished to prevent tongue irritation.
