Surgically Assisted Rapid Palatal Expansion (SARPE) combines a fixed palatal expander (typically a Hyrax) with surgical corticotomy cuts to the midpalatal suture and lateral maxillary walls, allowing expansion forces to overcome suture resistance in skeletally mature adults. After surgery the patient activates a jackscrew at prescribed intervals; a retention phase follows to stabilize new width. Indicated when non-surgical MARPE or MSE is predicted to fail due to complete skeletal maturation or Angelieri Stage D/E suture morphology.
