Overview

"Holding arch" is a clinical umbrella term for any fixed appliance that passively holds bilateral arch space after premature primary molar loss. In the upper arch this is typically a Nance holding arch (Nance holding arch); in the lower arch it is a lingual holding arch (Lingual holding arch / Lingual holding arch). The appliance has no active components — it simply prevents first permanent molars from drifting mesially into the space left by lost primary teeth, preserving room for erupting premolars.

Clinical & technical

Mechanism of action

Molar bands cemented to the first permanent molars connect to a wire framework running lingual to the arch. The wire contacts the anterior teeth (lower) or the palate (upper), providing a stop against mesial molar migration. Forces are passive and light — essentially preventing drift rather than generating active movement.

Indications & case selection

Bilateral premature loss of primary molars in either arch; Leeway space preservation in late mixed dentition to avoid crowding; holding space after extraction pending eruption of permanent successors; adjunct to comprehensive treatment planning when mixed-dentition space management is needed.

Contraindications & limitations

Passive only — cannot regain space already lost (use a space regainer Space regainer for that). Must be removed or replaced as permanent teeth erupt. Hygiene challenge — requires diligent flossing around the wire.
