Overview

Fixed space maintainers are cemented appliances prescribed when a primary tooth is lost prematurely before its permanent successor is ready to erupt. Without intervention the adjacent teeth drift into the space, reducing arch length and causing crowding, impaction, or malocclusion. The fixed family includes: band-and-loop (unilateral), lingual arch / Nance (bilateral), and distal shoe (pre-eruptive molar guidance). Clinical & technical

Mechanism of action

All fixed space maintainers work passively — they occupy and protect space without actively moving teeth. A cemented component (band, crown, or bonded pad) anchors the appliance to an adjacent tooth, and a wire extension or framework spans the edentulous space to block drifting of adjacent teeth. The critical dimension is that the wire extension must contact the tooth on the other side of the gap, preventing mesial or distal migration depending on which tooth was lost.

Indications & case selection

Premature loss of a primary tooth (caries, trauma, pathology) when the permanent successor is not expected to erupt within 6 months; cases where the space would be lost without intervention (confirmed by radiographic assessment of alveolar bone overlying the successor); bilateral loss (use bilateral devices — lingual arch or Nance); unilateral loss (use band-and-loop). Contraindicated in the rare case where early loss may actually benefit spacing or crowding management.

Contraindications & limitations

Not indicated if the permanent tooth is about to erupt (< 6 months of bone remaining). Requires monitoring — must be removed before the permanent tooth erupts or it will block eruption. Cannot replace a tooth or restore function — only maintains space. Poor oral hygiene leads to decay under bands and around the appliance. Fixed maintainers may inhibit normal alveolar development if left in too long.
