Overview

The lower tongue thrust appliance is a mandibular habit appliance designed to address cases where the tongue thrusts against the lower anterior teeth (rather than, or in addition to, the upper). It is based on the lower lingual arch framework (Lingual arch) with added crib or spur elements directed at the lingual surface of the lower anteriors. Less common than upper tongue cribs — most tongue thrusting primarily affects the upper arch.

Mechanism of action

Most tongue thrust habit appliances target the upper arch. The lower tongue thrust appliance instead acts on the mandibular arch, placing deterrent elements on the lingual surface of the lower anterior teeth. This is used when the tongue predominantly thrusts against or between the lower anterior teeth, or when lower arch instability from tongue posture is contributing to crowding or an open bite that is primarily mandibular.

Lab fabrication notes

The lower arch presents less space than the upper — all components must be minimal to avoid occlusal interference. Spur length is critical: too long and the patient cannot close comfortably; too short and the tongue adapts around them. Standard: 3–4 mm spurs positioned 2 mm lingual to the lower incisor contacts. The frenum relief in the midline is essential — carve a notch in the acrylic at least 5 mm wide and 3 mm deep at the frenum attachment area. Clasps on lower molars require care to engage the buccal undercut without impinging the mucosa.
